Training Background

Please complete the form below to give me a detailed sense of your history and background in sport.

What is your recreational and competitive sport background, and what is your background in swimming, cycling, running and triathlon?
For the past week, 3 weeks, and 3 months, how many days per week have you been devoting to each discipline (swim, bike, run) and any other regular training (gym, pilates, yoga, etc)?
What is your current long session [in hours/minutes for the bike and run, in meters for the swim] in the past week, 3 weeks, 3 months?
What are you doing for tempo or intensity in each of the three disciplines (past week, 3 weeks, 3 months)?

Do you have any current injuries?
What is your injury history?
Do you have any special concerns with respect to injury? Can you provide any additional information that would be helpful to me in protecting you from injury?
